I am a fee for service private practice provider and I do not directly accept insurance.  Although I do not bill your insurance directly, all or part of your services may qualify for reimbursement through your individual insurance plan.  I will provide you with a detailed invoice which can be used to submit to your insurance provider for potential out-of-network benefits. 

The following questions may be useful when speaking with your insurance plan representative: 

• Do I have out-of-network mental health insurance benefits?
• Do I need to reach a deductible, and if so, what is the deductible?
• What is the “usual and customary allowance?”
• What percentage do they cover of the “usual and customary allowance?”
